IZIN PENGGALIAN

Sebelum penggalian, lakukan pengecekan menyeluruh terhadap bagan lokasi dan gambar sesuai yang di bangun, untuk memastikan daerah galian bebas dari kabel- kabel atau instalasi di bawah tanah.

Sebelum penggalian, dapatkan izin yang sudah ditandatangani oleh Supervisor, yang memiliki wewenang akhir untuk pengeluaran surat izin penggalian.

Semua instalasi di bawah tanah harus ditandai/ diperlihatkan didalam surat izin dan tanahnya diberi tanda.

Jika diketahui ada kabel, pipa air atau ada gangguan lain di bawah tanah, maka dilakukan penggalian dengan tangan untuk menemukannya.

Jalan masuk dan keluar yang aman harus disediakan untuk semua penggalian.

Semua tempat penggalian harus diberi penghalang atau ditandai dengan pita peringatan.

Setelah penggalian selesai, izin harus ditutup oleh Supervisor.

IZIN MEMASUKI RUANG TERTUTUP

Sebelum memasuki ruang tertutup, Surat izin Masuk ke Ruang Tertutup harus dari HSE Coordinator. 

Rincian surat izin akan diisi oleh Supervisor.

HSE Coordinator akan mengadakan tes yang diperlukan dan menandatangani surat izin.

Setelah Supervisor puas dengan persyaratan keselamatan ia akan menandatangani surat izin dan mengizinkan pekerjaan untuk dimulai.

Setelah pekerjaan selesai, Supervisor akan menutup surat izin. Surat izin yang telah ditutup ini lalu disimpan oleh HSE Coordinator.

Apabila mungkin, ruang tertutup harus diberi ventilasi ke udara atau dengan ventilasi bertekanan udara (jangan gunakan oksigen untuk ventilasi).

Jika pengujian menunjukan adanya kekurangan oksigen, ruang tertutup harus dialiri udara segar sampai pengujian menyatakan tingkat oksigen sudah aman.

Jika pengujian menunjukkan kekurangan oksigen, ruang tertutup harus dialiri udara segar sampai pengujian menyatakan tingkat oksigen sudah aman.

Ruang tertutup yang telah dibersihkan dari gas yang mudah terbakar atau berbahaya harus diperiksa secara berkala untuk memastikan oksigen yang tersisa menunjukkan tingkat yang aman pada saat ada orang di dalamnya.

Jika keadaan memaksa seseorang untuk masuk ke ruang yang berudara tak aman, perlengkapan yang benar dan sesuai harus dipakai. Gunakan alat bantu pernapasan atau pemasok udara sendiri.

Dalam kasus seperti di atas, seorang “penyelamat”, yang juga di lengkapi dengan alat bantu pernapasan, harus siap di depan pintu masuk tertutup dalam kondisi ruangan seperti ini harus dilengkapi dengan sabuk pengaman dan tali pengaman dengan “penyelamat”.

Alat pendeteksi oksigen dan gas berbahaya harus disediakan dan siap pakai oleh HSE Coordinator setiap saat.

IZIN UNTUK KERJA PANAS 

(Dipakai untuk lingkungan yang mengandung bahaya)

Sebelum memulai pekerjaan panas di lingkungan yang mengandung bahaya tinggi, Izin Kerja Panas harus didapatkan dari HSE Coordinator atau Representatif Klien.

Hanya detail surat izin untuk giliran kerja saat itu yang akan diisi oleh supervisor lapangan.

HSE Coordinator or Representatif Klien akan mengadakan pengujian yang diperlukan dan menandatangani surat izin tersebut.

Setelah HSE Coordinator atau Reprensentatif Klien puas dengan persyaratan keselamatan ia akan menandatangani surat izin dan mengizinkan pekerjaan untuk dimulai.

Setelah pekerjaan atau giliran kerja selesai, Supervisor Lapangan akan menyatakan surat izin tersebut tidak berlaku.

Surat izin yang sudah tidak berlaku tersebut akan disimpan oleh HSE Coordinator.

Setiap giliran kerja harus memperoleh surat izin baru sesuai dengan prosedur seperti diatas.

Tidak dibenarkan melakukan panas setelah masa berlaku surat izin habis.

Jika dianggap perlu, HSE Coordinator harus memantau lapangan untuk mejamin daerah tersebut bebas dari gas saat pekerjaan sedang berlangsung.

PERMIT TO WORK SYSTEM

EXCAVATION PERMIT

Prior to any excavation, a thorough review of the site plan and as built drawings must be made to ensure the area is free from service lines or cables.

Prior to any excavation, a permit must be obtained and signed off by Supervisor who have the final authority for permits.

All known underground services must be clearly indicated on the permit and stake out on the ground.

If underground cables, waterline or other obstacles are known to exist, they will be located by hand digging.

Safe access and egress must be provided for all excavations.

All excavations shall be barricaded or marked with warning tapes or bunting.

Following completion of the work, the permit should be closed out by the Supervisor.

CONFINED SPACE ENTRY PERMIT

9.2.1 Prior to entry to a confined space, a Confined Space Entry Permit will be obtained the HSE Coordinatoe.

The supervisor will fill in details of the permit.

HSE Coordinator conduct required tests and sign the permit.

Supervisor, having satisfied himself all Safety requirements have been met, will sign the permit and authorise the work to commence.

At the completion of the work, Supervisor will sign off the permit. Closed out permit will be filed by HSE Coordinator.

Whenever possible, the confined space shall be vented to atmosphere of force ventilate with air movers. (Oxygen should not be used for ventilation).

Prior to personnel entering confined spaces, a test shall be performed by safety personnel for the presence of dangerous or combustible gases.

If test indicates a deficiency in oxygen content, the space shall be ventilate with fresh air until tests indicated the oxygen level is safe.

Confined spaces which have been purged of combustible or dangerous gases shall be checked periodically to ensure the oxygen level remains safe while personnel remain in the space.

If it is necessary to enter a confined space, which does not have a safe atmosphere, the correct level of protective equipment must be used. This would include self-contained or supplied air breathing apparatus.

In such cases a “rescuer”, also equipped with apparatus, shall be present at the entrance to the confined space. Personnel entering the confined space under such conditions shall be fitted with a safety belt and lift line to the standby “rescuer”.

The HSE Coordinator shall have readily available at all times, a fully serviceable combustible gas detector.

HOT WORK PERMIT

(Use in potentially hazardous environment).

Prior to commencing any hot work in a potentially hazardous environment, a Hot Work Permit shall be obtained from the HSE Coordinator or Client Representative.

Details of the permit for that shift only will fill out by the worksite Supervisor.

HSE Coordinator or Client representative will conduct the required tests and sign the permit.

HSE Coordinator or Client Representative, having satisfied himself that all the Safety requirements have been met, will sign the permit and authorise the work to commence.

At the completion of the work or shift, the Worksite Supervisor will sign off the permit.

Closed out permits will be filed by HSE Coordinator.

Each incoming shift shall apply for a new permit as per the above procedure.

No hot work shall be performed beyond the expiry time stated on the Hot Work Permit.

Where necessary, HSE Coordinator shall monitor the area to maintain its gas free status while work is progressing.
